UNICEF &ndash; Bolivia. </font></p> <hr>     <div align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">El programa sobre la Atenci&oacute;n Integral de la Enfermedades Prevalentes de la Infancia (AIEPI) es una estrategia, cuyos componentes se orientan a: mejorar la capacidad del personal de salud para la promoci&oacute;n de la salud y atenci&oacute;n a enfermedades, mejorar las pr&aacute;cticas familiares y comunitarias, etc. Desde hace m&aacute;s de una d&eacute;cada, esta estrategia ha contribuido de sobre manera, en la reducci&oacute;n de la morbi-mortalidad de muchos pa&iacute;ses, incluida Bolivia. </font> </div>     <P align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif"> El AIEPI cl&iacute;nico, brinda una metodolog&iacute;a integradora y sistem&aacute;tica para la atenci&oacute;n del ni&ntilde;o/a menor de 5 a&ntilde;os. Actualmente, el Ministerio de Salud y Deportes ha trabajado en la adecuaci&oacute;n del AIEPI-Nutricional cl&iacute;nico para que sea coherente con el plan de &ldquo;Desnutrici&oacute;n cero&rdquo;, incorporando elementos novedosos, basados en la evidencia cient&iacute;&#64257;ca, para la prevenci&oacute;n y tratamiento de la desnutrici&oacute;n, as&iacute; como de las enfermedades prevalentes. </font></P>     <P align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif"> Una de las principales modi&#64257;caciones es la incorporaci&oacute;n de indicadores peso para la talla y talla para la edad, en el diagn&oacute;stico de la desnutrici&oacute;n, tomando como referencia a los nuevos patrones de crecimiento de la Organizaci&oacute;n Mundial de la Salud (OMS), lanzados a nivel mundial en abril del presente a&ntilde;o; por lo que Bolivia se convierte en el primer pa&iacute;s de la regi&oacute;n en  adoptar estos nuevos est&aacute;ndares de crecimiento. </font></P>     <P align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif"> Otra adecuaci&oacute;n consiste en la inclusi&oacute;n del zinc como parte del tratamiento estandarizado de la diarrea, en todas sus formas. Las evidencias cient&iacute;&#64257;cas, en relaci&oacute;n al uso de este micronutriente, son contundentes<sup>1-4</sup>, disminuye la duraci&oacute;n del episodio diarreico y previene la aparici&oacute;n de eventos futuros. La UNICEF y la OMS recomendaron su uso en mayo del 2004 a&#64257;rmando que se podr&iacute;an disminuir las muertes por diarrea hasta en un 20%, si se usa de forma generalizada<sup>1</sup>. </font></P>     <P align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif"> Las adecuaciones, en el tema de la diarrea, consideran el empleo de sales de rehidrataci&oacute;n oral (SRO) de osmolaridad reducida con sodio 75mEq/L y osmolaridad 245mosm/L; que en la actualidad es otro elemento indispensable para el manejo correcto de las diarreas, demostrando fehacientemente en muchos ensayos cl&iacute;nicos que es superior a las sales est&aacute;ndar (sodio 90mEq/L y 311mosm/L) en el manejo de diarrea aguda, principalmente no col&eacute;rica<sup>5-8</sup>. Esto gracias a la reducci&oacute;n del gasto fecal y disminuci&oacute;n hasta en un 33% de la necesidad de soluciones endovenosas en ni&ntilde;os manejados con estas nuevas SRO. </font></P>     ]]></body>
<body><![CDATA[<P align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif"> Otra novedad, incorporada en AIEPI-Nutricional, consiste en el empleo de la cipro&#64258;oxacina oral para el manejo de la disenter&iacute;a<sup>9-11</sup>, utilizando esta &#64258;uoroquinolona durante un curso corto de 3 d&iacute;as. Las justi&#64257;caciones para esta recomendaci&oacute;n son diversas, entre ellas se destacan: la resistencia casi generalizada de la shigella al cotrimoxazol, el desarrollo de resistencia r&aacute;pida y cruzada al &aacute;cido nalid&iacute;xico y la disponibilidad casi nula de otras opciones por costos o razones operativas como son: ceftriaxona y azitromicina. </font></P>     <P align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif"> Consideramos que si estas innovaciones son aplicadas masivamente y de forma racional, existir&aacute; una reducci&oacute;n considerable no solo de la desnutrici&oacute;n, si no de la morbimortalidad por diarrea y neumon&iacute;a, principales causa de muerte de los ni&ntilde;os y ni&ntilde;as bolivianos. </font></P>     <P align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif"> Es importante el compromiso y apoyo de la Sociedad Boliviana de Pediatr&iacute;a y de todos los pediatras, hacia la meta &ldquo;Desnutrici&oacute;n Cero&rdquo; y particularmente hacia la aplicaci&oacute;n y difusi&oacute;n del AIEPI-Nutricional cl&iacute;nico.
