Obtener el diagnóstico de situación en relación a las verdaderas causas de mortalidad infantil inesperada en la ciudad de Montevideo. 3. Realizar el diagnóstico de situación y posicionamiento real del síndrome de muerte súbita del lactante dentro de las muertes inesperadas. Material y método: a los lactantes fallecidos inesperadamente en Montevideo, en el período comprendido entre octubre de 1998 y marzo de 2000 se les aplicó sistemáticamente: 1. Autopsia completa protocolizada, con fotografía, radiología, toma de muestras para enfermedades metabólicas, microbiología, toxicología y estudio bioquímico (ionograma del humor vítreo). 2. Se realizó histología completa de todos los órganos. 3. Se investigaron las circunstancias que rodearon a la muerte y se revisó la historia clínica. 4. Se discutió cada caso en grupo multidisciplinario. Resultados: se estudiaron 115 casos de los cuales 109 (95%) correspondieron a menores de un año y 6 a menores entre los 12 y 18 meses. En 65% de los casos se confirmó una causa de muerte. Correspondieron al grupo de Muerte Explicable. Dentro de este grupo la infección respiratoria baja, las causas cardíacas, la diarrea con deshidratación y la sepsis constituyeron las causas más importantes. El síndrome de muerte súbita del lactante representó el 20% de la muestra y la zona gris el 15%. Conclusiones: la realización sistemática de la autopsia protocolizada por patólogo pediatra y forense ha permitido conocer la verdadera causa de muerte en los lactantes de Montevideo que han fallecido inesperadamente. El desarrollo del programa Muerte Inesperada del Lactante ha permitido tomar un tema que en el Uruguay estaba limitado a los archivos judiciales y ponerlo en la mesa de discusión pediátrica para su aplicación en la docencia, investigación y planificación de políticas de salud, siguiendo los criterios actualmente sugeridos por la literatura internacional.]]></p></abstract>
<abstract abstract-type="short" xml:lang="en"><p><![CDATA[Objectives: 1. To apply a uniform autopsy protocol to study all infants who die suddenly and unexpectedly in Montevideo. 2. To know the prevalence of the causes of death presenting initially as sudden and unexpected. 3. To identified among those, the cases of true sudden infant death syndrome (SIDS). Methods: autopsies were performed in infants dying suddenly and unexpectedly from October 1998 to March 2000. The study was conducted by a pediatric pathology (PP) and an expert in forensic science and included complete macroscopic and histologic examination, photography, radiology, toxicology, microbiology, vitreous electrolyte measurement, clinical history review and death scene investigation. Final diagnosis conferences were held with PPs, pediatricians and pediatric neurologists. Results: the number of deaths investigated was 115. The age range was extended to include infants up to 18 months, but 109 cases (95%) were younger than 1 year and 6 cases (5%) were 12 to 18 months old. 65% of the cases were non-SIDS, or explained deaths. Pulmonary infections, cardiac anomalies, dehydration and septicemia were the most important causes of death. 20% of the cases were SIDS and 15% of the cases were unexplained or undetermined (gray zone). Conclusions: the development of this program has allowed a clear understanding of the causes of death in infancy in Montevideo and it provided the means for health care improvement. It also contributed to the education of health care providers since it was possible to discuss these cases, previously seen only by forensic and legal personnel, at conferences widely attended by physicians of different specialties involved in the care of infants. The information obtained through this program will be used in preventing explained deaths as a way to lower infant mortality rate in Uruguay.]]></p></abstract>

<body><![CDATA[<p align="justify"><font face="Verdana" size="2">Se define al s&iacute;ndrome de muerte s&uacute;bita en la infancia (SMSI) como la muerte repentina e inesperada de un ni&ntilde;o menor de un a&ntilde;o de edad que permanece sin explicaci&oacute;n luego de una profunda investigaci&oacute;n que incluye el estudio necr&oacute;psico completo, el examen de la escena de la muerte y la revisi&oacute;n de la historia cl&iacute;nica<sup>(1)</sup>. </font></p>     <p align="justify"><font face="Verdana" size="2">Dos estudios realizados por el mismo grupo<sup>(2,3)</sup>, en la ciudad de Montevideo en el a&ntilde;o 1996, mostraron por un lado que la mitad de los ni&ntilde;os fallecidos menores de un a&ntilde;o (pasado el per&iacute;odo neonatal) lo hac&iacute;an en su casa y sin asistencia m&eacute;dica; por otro lado los peritajes realizados en la morgue judicial a esa poblaci&oacute;n (N=122) mostraron un 96% de muertes naturales y un 4%de muertes violentas. De las muertes que se consideraron "naturales", los autores encontraron con mayor frecuencia la infecci&oacute;n respiratoria (32% de los casos), aspiraci&oacute;n de v&oacute;mito (24,6%), s&iacute;ndrome de muerte s&uacute;bita del lactante (15,6%) e indeterminadas en 13,1%. Analizando la situaci&oacute;n en Uruguay, otros autores la objetivaron como un problema sanitario no resuelto con connotaciones jur&iacute;dicas que deben contemplarse, y plantearon la necesidad del abordaje multidisciplinario del tema con la creaci&oacute;n de un equipo de estudio nacional que contemple en su integraci&oacute;n el &aacute;mbito pedi&aacute;trico, an&aacute;tomo-patol&oacute;gico y m&eacute;dico legal y que permita formular pol&iacute;ticas sanitarias v&aacute;lidas<sup>(4)</sup>. Los dos grupos coinciden en que la metodolog&iacute;a diagn&oacute;stica hasta ese momento no segu&iacute;a los requerimientos internacionales. </font></p>     <p align="justify"><font face="Verdana" size="2">Mediante un acuerdo entre el Ministerio de Salud P&uacute;blica (MSP), el Centro Hospitalario Pereira Rossell (CHPR), el Poder Judicial y la Sociedad Uruguaya de Pediatr&iacute;a, se pudo desarrollar el programa MIL (Muerte Inesperada del Lactante) por el cual se realizan en el Laboratorio de Patolog&iacute;a Pedi&aacute;trica del CHPR las autopsias de los ni&ntilde;os menores de un a&ntilde;o fallecidos en domicilio o a su ingreso al hospital. Este programa se inici&oacute; el 1 de octubre de 1998. El equipo de pat&oacute;logos pediatras, junto con el m&eacute;dico forense, realizan las autopsias con un protocolo sistem&aacute;tico que incluye examen macrosc&oacute;pico externo con fotograf&iacute;a, radiolog&iacute;a del esqueleto, estudio histol&oacute;gico completo, toma de muestras para ex&aacute;menes microbiol&oacute;gicos, toxicol&oacute;gicos, metab&oacute;licos, as&iacute; como estudio del humor v&iacute;treo. Obtenidos estos resultados se realiza, en algunos casos, la visita domiciliaria y se llega a un diagn&oacute;stico final que incluye: diagn&oacute;stico an&aacute;tomo-patol&oacute;gico de la autopsia, conocimiento de las circunstancias que rodearon a la muerte y discusi&oacute;n en grupo multidisciplinario por los integrantes del Grupode Estudio que se integra como Comit&eacute; de la Sociedad Uruguaya de Pediatr&iacute;a. &Eacute;sta es una reuni&oacute;n integrada por pediatras, neuropediatras, especialista en enfermedades metab&oacute;licas, genetista, forense y pat&oacute;logos pediatras. Se elabora, de esa manera, el diagn&oacute;stico final que es el resultado de una discusi&oacute;n multidisciplinaria. Se realizan reuniones cl&iacute;nico-patol&oacute;gicas en los Centros de Salud para devolver la informaci&oacute;n con los resultados de las autopsias, as&iacute; como con los diferentes servicios pedi&aacute;tricos del CHPR donde estos ni&ntilde;os estuvieron alguna vez internados. </font></p>     <p align="justify"><font face="Verdana" size="2">El objetivo de este estudio es: </font></p> <ol>       <li><font size="2" face="Verdana" align="justify">Aplicar un protocolo de estudio uniforme a todos los lactantes fallecidos en forma inesperada en la ciudad de Montevideo.</font></li>       <li><font face="Verdana" size="2">Obtener el diagn&oacute;stico de situaci&oacute;n en relaci&oacute;n a las verdaderas causas de mortalidad infantil inesperada en la ciudad de Montevideo. </font></li>       <li><font face="Verdana" size="2"> Realizar el diagn&oacute;stico de situaci&oacute;n y posicionamiento real del s&iacute;ndrome de muerte s&uacute;bita del lactante (SMSL) dentro de las muertes inesperadas. </font>   </li>     </ol> <font face="Verdana" size="2"><b></b></font>     <p align="justify"><font size="3" face="Verdana"><b>    <br> Material y m&eacute;todo</b></font></p>     ]]></body>
<body><![CDATA[<p align="justify"><font face="Verdana" size="2">La poblaci&oacute;n estudiada correspondi&oacute; a todos los menores de un a&ntilde;o fallecidos en domicilio o a su ingreso al hospital. Se incluyeron adem&aacute;s casos entre 12 y 18 meses fallecidos inesperadamente en iguales circunstancias y que fueron enviados para su estudio. </font></p>     <p align="justify"><font face="Verdana" size="2">Las autopsias se realizaron en conjunto por pat&oacute;logo pediatra y m&eacute;dico forense en el Laboratorio de Patolog&iacute;a Pedi&aacute;trica del CHPR. </font></p>     <p align="justify"><font face="Verdana" size="2">A todos los casos se les practic&oacute; un examen externo minucioso, con documentaci&oacute;n fotogr&aacute;fica externa obligatoria (e interna de toda alteraci&oacute;n encontrada). </font></p>     <p align="justify"><font face="Verdana" size="2">Se realiz&oacute; radiolog&iacute;a del esqueleto de frente y de perfil en el Departamento de Imagenolog&iacute;a del CHPR con el objetivo de investigar fracturas &oacute;seas u otra lesi&oacute;n de etiolog&iacute;a traum&aacute;tica previa y la determinaci&oacute;n de sacar muestras para histolog&iacute;a de las &aacute;reas &oacute;seas lesionadas a efectos de datar el proceso. </font></p>     <p align="justify"><font face="Verdana" size="2">Se tomaron muestras de rutina para ex&aacute;menes microbiol&oacute;gicos de pulm&oacute;n y sangre que se estudiaron en el Laboratorio de Bacteriolog&iacute;a del CHPR. En los casos en que se encuentran en la autopsia procesos lesionales con colecciones anormales en otras topograf&iacute;as, tambi&eacute;n se tomaron muestras de las mismas. </font></p>     <p align="justify"><font face="Verdana" size="2">Se tomaron muestras para estudios toxicol&oacute;gicos de sangre, bilis y orina para su estudio en el Laboratorio de Toxicolog&iacute;a del Instituto T&eacute;cnico Forense. </font></p>     <p align="justify"><font face="Verdana" size="2">Se recogieron muestras para investigaci&oacute;n de enfermedades metab&oacute;licas que se almacenaron en nitr&oacute;geno l&iacute;quido (m&uacute;sculo e h&iacute;gado) y en <em>freezer</em> (bilis, gota de sangre y humor v&iacute;treo). </font></p>     <p align="justify"><font face="Verdana" size="2">Se conserv&oacute; en nitr&oacute;geno una muestra de pulm&oacute;n para congelaci&oacute;n y determinaci&oacute;n de lip&oacute;fagos con t&eacute;cnicas para grasa. </font></p>     <p align="justify"><font face="Verdana" size="2">Se envi&oacute; humor v&iacute;treo al Laboratorio de Urgencia del CHPR para estudio bioqu&iacute;mico (ionograma del humor v&iacute;treo). </font></p>     <p align="justify"><font face="Verdana" size="2">Se realiz&oacute; una disecci&oacute;n cuidadosa con examen macrosc&oacute;pico minucioso de todos los &oacute;rganos y las diferentes estructuras y determinaci&oacute;n de pesos viscerales. </font></p>     ]]></body>
<body><![CDATA[<p align="justify"><font face="Verdana" size="2">Se catalogaron como desnutridos a los que ten&iacute;an un peso corporal inferior o igual al percentil 5 para la edad. </font></p>     <p align="justify"><font face="Verdana" size="2">Se realiz&oacute; estudio histol&oacute;gico completo y generoso de todos los &oacute;rganos: aparato respiratorio completo, con fragmentos pulmonares de cada l&oacute;bulo, incluidos la zona hiliar y la periferia. En todos los casos, adem&aacute;s del estudio de rutina con hematoxilina y eosina (HE), se realiz&oacute; coloraci&oacute;n de Perls para ver sider&oacute;fagos y coloraci&oacute;n de Grocott para investigar hongos y <em>Pneumocistis carinii</em>. Se tomaron m&uacute;ltiples fragmentos de coraz&oacute;n incluidos miocardio ventricular parietal bilateral y septal a varios niveles: cortes de m&uacute;sculos papilares y de coronarias, as&iacute; como de los vasos de la base. En casos seleccionados se realiz&oacute; estudio de sistema de conducci&oacute;n. Se tomaron cortes del tubo digestivo desde la lengua hasta la margen anal, incluidas las gl&aacute;ndulas anexas con cortes de la totalidad del p&aacute;ncreas. Se realiz&oacute; histolog&iacute;a de todos los &oacute;rganos endocrinos, as&iacute; como estudio del sistema nervioso de acuerdo a la sistem&aacute;tica aplicada en autopsia pedi&aacute;trica incluido el estudio de m&eacute;dula espinal. </font></p>     <p align="justify"><font face="Verdana" size="2">Adem&aacute;s de la coloraci&oacute;n de HE se realizaron t&eacute;cnicas para mielina (Luxol Fast Blue). Se estudiaron los &oacute;rganos linfoides, incluida la m&eacute;dula &oacute;sea. Se estudiaron m&uacute;sculo, hueso (uni&oacute;n condrocostal y v&eacute;rtebra), o&iacute;do y globo ocular. </font></p>     <p align="justify"><font face="Verdana" size="2">En el presente estudio se define como medio socioecon&oacute;mico deficitario a los casos que tienen certificado de precariedad expedido por la municipalidad. </font></p>     <p align="justify"><font face="Verdana" size="2">Se realizaron visitas domiciliarias a los hogares de los ni&ntilde;os sin causa de muerte por la autopsia para recoger datos acerca de las circunstancias que rodearon dicho deceso. </font></p>     <p align="justify"><font face="Verdana" size="2">Se dise&ntilde;&oacute; previamente un protocolo de interrogatorio. </font></p>     <p align="justify"><font face="Verdana" size="2">Se revisaron las historias cl&iacute;nicas de los pacientes. Se analiz&oacute; cada caso, en grupo multidisciplinario, por los integrantes del Grupo de Estudio que se integr&oacute; como Comit&eacute; de la Sociedad Uruguaya de Pediatr&iacute;a. &Eacute;sta es una reuni&oacute;n integrada por pediatras, neuropediatras, especialista en enfermedades metab&oacute;licas, forense y pat&oacute;logos pediatras. </font></p>     <p align="justify"><font face="Verdana" size="2">El equipo de estudio realiza reuniones cl&iacute;nico-patol&oacute;gicas con los Centros de Salud de cada zona para devolver la informaci&oacute;n con los resultados de las autopsias. Tambi&eacute;n se realizan reuniones de retorno con los diferentes servicios pedi&aacute;tricos del CHPR donde estos ni&ntilde;os estuvieron alguna vez internados. </font><font face="Verdana" size="2"><b> </b></font></p> <font face="Verdana" size="2"><b></b></font>     <p align="justify"><font size="3" face="Verdana"><b>    <br> Resultados </b></font></p>     ]]></body>
<body><![CDATA[<p align="justify"><font face="Verdana" size="2">Se estudiaron 109 casos de muerte inesperada en lactantes menores de un a&ntilde;o en el per&iacute;odo comprendido entre el 1&deg; de octubre de 1998 y el 31 de marzo de 2000. Se agregaron adem&aacute;s seis casos entre 12 y 18 meses. El total de casos fue 115: 94,8% menores de un a&ntilde;o, 75% menores de seis meses; 50% entre uno y cuatro meses, 17% menores de un mes (<a href="#t1">tabla 1</a>).</font></p>     <p align="center"><a name="t1"></a><img src="/img/revistas/rbp/v42n2/tabla_a13_1.gif" width="316" height="177"></p>     <p align="justify"><font face="Verdana" size="2">En relaci&oacute;n al sexo, fue masculino en 68 (59%) y femenino en 47 casos (41%) (<a href="#t2">tabla 2</a>).</font></p> <font face="Verdana" size="2"><b></b></font><font face="Verdana" size="2"><b>     <p align="center"><a name="t2"></a><img src="/img/revistas/rbp/v42n2/tabla_a13_2.gif" width="319" height="74"></p> </b></font>     <p align="justify"><font face="Verdana" size="2">El medio socioecon&oacute;mico fue deficitario en 77 casos (67%), aceptable en 14 casos (12,2%), bueno en nueve casos (7,8%) y no se dispuso de informaci&oacute;n en 15 casos (13%) (<a href="#t3">tabla 3</a>). </font></p> <font face="Verdana" size="2"><b>     <p align="center"><a name="t3"></a><img src="/img/revistas/rbp/v42n2/tabla_a13_3.gif" width="319" height="110"></p> </b></font>     <p align="justify"><font face="Verdana" size="2">La atenci&oacute;n del parto fue: en instituciones p&uacute;blicas 79 casos (68,7%), en privadas 13 casos (11,3%) y no dispusimos de informaci&oacute;n en 23 casos (20%). El 76,2% de los ni&ntilde;os de esta muestra naci&oacute; en el CHPR. </font></p>     <p align="justify"><font face="Verdana" size="2">El peso al nacer fue adecuado (mayor a 2.500 g) en 69 casos (60%), entre 2.000 y 2.500 g en 12 casos (10,4%), entre 1.500 y 2.000 g en siete casos (6,1%) y menor de 1.500 g en cinco casos (4,3%). No dispusimos de informaci&oacute;n en 22 casos (19,1%) (<a href="#t4">tabla 4</a>). </font></p> <font face="Verdana" size="2"><b>     <p align="center"><a name="t4"></a><img src="/img/revistas/rbp/v42n2/tabla_a13_4.gif" width="319" height="127"></p> </b></font>     <p align="justify"><font face="Verdana" size="2">Los domicilios de los fallecidos correspondieron a las siguientes seccionales policiales: 58 casos (50,4%) al grupo constituido por las seccionales 12, 17, 18, 19 y 24, que son zonas mayoritariamente carenciadas. Otro grupo, constituido por las seccionales 1,4 y 5 (que nuclean importante n&uacute;mero de casas de inquilinato), dio cuenta de 18 casos (15,6%). Luego la seccional 15 (correspondiente a Malv&iacute;n Norte) tuvo seis casos (5,2%). En la franja costera, que engloba a barrios residenciales, no se registraron casos. </font></p>     ]]></body>
<body><![CDATA[<p align="justify"><font face="Verdana" size="2">La autopsia se realiz&oacute; siempre antes de las 48 horas del fallecimiento, pero en 56% de los casos luego de las 24 horas (<a href="#t5">tabla 5</a>).</font></p>     <p align="center"><font size="2" face="Verdana"><a name="t5"></a><img src="/img/revistas/rbp/v42n2/tabla_a13_5.gif" width="318" height="107"></font></p>     <p align="justify"><font face="Verdana" size="2">Se realizaron estudios metab&oacute;licos para investigaci&oacute;n de trastornos de la beta-oxidaci&oacute;n en cinco casos, que fueron negativos. </font></p>     <p align="justify"><font face="Verdana" size="2">De los 115 casos examinados, se encontr&oacute; una causa que justificaba la muerte en 75 de ellos (65,2%), es el grupo que se identifica como <strong>muerte explicable</strong> (ME). En 23 casos (20%) no se encontraron alteraciones en la autopsia que justificaran la muerte, este es el grupo que se identifica como <strong>autopsia negativa</strong> (AN). Son los casos que, consideradas las circunstancias que rodearon a la muerte y la historia cl&iacute;nica, constituyen el grupo de SMSL. En 17 casos (14,8%) se encontraron patolog&iacute;as aunque insuficientes en s&iacute; mismas para explicar la muerte. &Eacute;ste es el grupo que se identifica como <strong>zona gris</strong> (ZG) y que otros autores tipifican como "indeterminado", "indefinido", "cuestionado", "posible...". Lo importante es que</font><font face="Verdana" size="2"><b> </b></font><font face="Verdana" size="2">las observaciones en todos estos casos fueron registradas de manera est&aacute;ndar y se ajustan a los criterios de grupos internacionales dedicados al tema (<a href="#t6">tabla 6</a>). </font></p>     <p align="center"><a name="t6"></a><img src="/img/revistas/rbp/v42n2/tabla_a13_6.gif" width="323" height="92"></p>     <p align="justify"><font face="Verdana" size="2">Las enfermedades encontradas como causa de muerte en el grupo de muerte explicable se detallan en la (<a href="#t7">tabla 7</a>). </font></p>     <p align="center"><a name="t7"></a><img src="/img/revistas/rbp/v42n2/tabla_a13_7.gif" width="320" height="481"></p>     <p align="justify"><font face="Verdana" size="2">Procesos m&oacute;rbidos asociados al grupo de muerte explicable: en este grupo se encontraron hallazgos lesionales de menor entidad que no constituyeron por s&iacute; solos la causa de muerte sino asociaciones (<a href="#t8">tabla 8</a>). </font></p>     <p align="center"><a name="t8"></a><img src="/img/revistas/rbp/v42n2/tabla_a13_8.gif" width="308" height="284"></p>     <p align="justify"><font face="Verdana" size="2">La zona gris estuvo constituida por 17 casos. En este grupo se encontraron patolog&iacute;as, pero insuficientes para causar la muerte (<a href="#t9">tabla 9</a>).</font></p>     ]]></body>
<body><![CDATA[<p align="center"><a name="t9"></a><img src="/img/revistas/rbp/v42n2/tabla_a13_9.gif" width="310" height="241"></p>     <p align="justify"><font face="Verdana" size="2">En el grupo tipificado como autopsia negativa, constituido por 23 pacientes, se encontraron algunas alteraciones menores que no invalidaron su consideraci&oacute;n dentro del <strong>SMSL</strong>. Se destaca el hallazgo de granulomas hep&aacute;ticos sin identificaci&oacute;n de g&eacute;rmenes en cuatro casos y dos casos de aspiraci&oacute;n de v&oacute;mito. </font></p> <font face="Verdana" size="2"><b></b></font><font face="Verdana" size="2"><b> </b></font>     <p align="justify"><font size="3" face="Verdana"><b>    <br> Comentarios</b></font></p>     <p align="justify"><font face="Verdana" size="2">En los pa&iacute;ses del primer mundo el SMSL es la primera causa de mortalidad infantil excluida la postnatal inmediata<sup>(5-7)</sup>. El conocimiento de la situaci&oacute;n en esos lugares ha permitido actuar mejor sobre los factores de riesgo de una manera preventiva<sup>(8,9)</sup>. </font></p>     <p align="justify"><font face="Verdana" size="2">Atrav&eacute;s del programa MIL se practica no solo un protocolo uniforme que se ajusta a los requerimientos del protocolo internacional sino que se logra la acci&oacute;n coordinada y conjunta del m&eacute;dico forense con su conocimiento en la muerte violenta<sup>(10-13)</sup> y la del pat&oacute;logo pediatra con su formaci&oacute;n en las enfermedades propias del ni&ntilde;o. La discusi&oacute;n multidisciplinaria final, con el conocimiento de las circunstancias que rodean a la muerte, permite una conclusi&oacute;n de la verdadera causa de la muerte, lo que es &uacute;til no s&oacute;lo desde el punto de vista jur&iacute;dico sino tambi&eacute;n m&eacute;dico, con resultados &uacute;tiles para la pediatr&iacute;a y las pol&iacute;ticas de salud en la infancia. </font></p>     <p align="justify"><font face="Verdana" size="2">La revisi&oacute;n de los casos incluidos en esta presentaci&oacute;n permite realizar las siguientes consideraciones: </font></p> <ul>       <li><font size="2" face="Verdana" align="justify">En relaci&oacute;n al sexo, predomina el masculino como en las dem&aacute;s series de la literatura. </font></li>       <li>         <div align="justify"><font face="Verdana" size="2"> En relaci&oacute;n a la edad, es llamativo el n&uacute;mero elevado de neonatos fallecidos en domicilio comprendidos en la presente serie. Especialmente el hecho de que 50% de los casos deANcorrespondan a muertes domiciliarias de neonatos. </font></div>   </li>       ]]></body>
<body><![CDATA[<li>         <div align="justify"><font face="Verdana" size="2"> El medio socio-econ&oacute;mico predominantemente deficitario unido a un elevado porcentaje de muertes explicables, as&iacute; como la ausencia de casos en la franja costera permiten una reflexi&oacute;n acerca de la posibilidad de que se est&eacute;n firmando certificados de defunci&oacute;n por complacencia en casos de muertes inesperadas de lactantes en domicilio. Esto estar&iacute;a hablando de una "cifra oscura" de la mortalidad infantil. </font></div>   </li>       <li>         <div align="justify"><font face="Verdana" size="2"> El an&aacute;lisis del peso al nacer y el estado nutricional al morir muestra que m&aacute;s del 70% de los ni&ntilde;os nacen con un peso superior a los 2.000 g, considerado con buen potencial para vivir, sin embargo 21% est&aacute; desnutrido al morir. </font></div>   </li>       <li>         <div align="justify"><font face="Verdana" size="2"> En 56% de los casos el retardo superior a 24 horas en realizar la autopsia se debi&oacute; a que por razones culturales se realiz&oacute; el velatorio previo en muchos casos. Una vez llegado el caso a la morgue del CHPR, el equipo de pat&oacute;logo y forense se re&uacute;ne para realizar la autopsia. La consecuencia indeseable de la captaci&oacute;n tard&iacute;a es que modifica los resultados de los cultivos bacteriol&oacute;gicos, proliferando una flora polimicrobiana como fen&oacute;meno post-mortem. </font></div>   </li>       <li>         <div align="justify"><font face="Verdana" size="2"> Es importante considerar el hecho de que esta poblaci&oacute;n abarca un per&iacute;odo de 18 meses constituido por: dos veranos, dos oto&ntilde;os y un invierno. Deber&aacute; tenerse en cuenta este dato al valorar la incidencia relativa de las diferentes patolog&iacute;as. </font></div>   </li>       <li>         <div align="justify"><font face="Verdana" size="2"> La mayor&iacute;a de los casos corresponden a ME (65%). La infecci&oacute;n respiratoria aguda constituy&oacute; la causa de muerte en un importante n&uacute;mero de casos. El elevado n&uacute;mero de casos permite plantear, como interrogante, la posibilidad de que los s&iacute;ntomas de enfermedad hayan sido subestimados, la manifestaci&oacute;n cl&iacute;nica haya sido escasa o la evoluci&oacute;n del proceso haya sido r&aacute;pida. </font></div>   </li>       ]]></body>
<body><![CDATA[<li>         <div align="justify"><font face="Verdana" size="2"> En un n&uacute;mero importante de casos la muerte fue por diarrea con deshidrataci&oacute;n y trastornos hidroelectro l&iacute;ticos, sobre todo hiponatremias severas, sin poder definir concretamente en cada caso la raz&oacute;n del descenso del sodio, se sabe que la p&eacute;rdida de agua desde el espacio extracelular agrava el trastorno circulatorio, empeorando la situaci&oacute;n de deshidrataci&oacute;n<sup>(14)</sup>.</font></div>   </li>       <li>         <div align="justify"><font face="Verdana" size="2">Algunos ni&ntilde;os fallecidos en el curso de la deshidrataci&oacute;n ten&iacute;an evidencias de diarrea (heces l&iacute;quidas en el colon y colitis en el estudio an&aacute;tomo-patol&oacute;gico). Sin embargo los s&iacute;ntomas no fueron considerados. </font></div>   </li>       <li>         <div align="justify"><font face="Verdana" size="2"> La enfermedad metab&oacute;lica puede dar cuenta de hasta un 10% de los casos de muerte inesperada de los lactantes<sup>(15-18)</sup>. </font></div>   </li>     </ul>     <p align="justify"><font face="Verdana" size="2">Se han descrito varias enfermedades que potencialmente son capaces de tener esta evoluci&oacute;n<sup>(19-22)</sup>, particularmente los trastornos de la oxidaci&oacute;n de los &aacute;cidos grasos<sup>(23-25)</sup>. La deficiencia de Acil-CoA deshidrogenasa de cadena media es la m&aacute;s frecuente. En un 90% hay una mutaci&oacute;n A985G<sup>(15,17)</sup> que puede estudiarse en sangre seca conservada en papel de filtro. Hay normas para recolecci&oacute;n de muestras destinadas a estudios metab&oacute;licos<sup>(17)</sup>. Si bien se deben recoger muestras de todos los pacientes, no est&aacute; justificado su estudio en todos los casos<sup>(26)</sup>. Se debe considerar la historia del paciente, la genealog&iacute;a y el antecedente de hermano fallecido<sup>(16)</sup> y el hallazgo de esteatosis hep&aacute;tica, mioc&aacute;rdica o muscular as&iacute; como cambios mitocondriales<sup>(17,18,26)</sup>. El diagn&oacute;stico fundamental para el asesoramiento, la prevenci&oacute;n y tratamiento de la hermandad<sup>(21)</sup>. En la presente serie realiz&oacute; el estudio metab&oacute;lico en tejido hep&aacute;tico de 6 casos cuyo resultado fue negativo, conserv&aacute;ndose las restantes muestras. </font></p>     <p align="justify"><font face="Verdana" size="2">De acuerdo a las diferentes series un porcentaje variable de muertes inesperadas del lactante (MIL) tienen causa reconocida<sup>(16,35-41)</sup>. En general coinciden en que la causa violenta no supera el 3% de los casos. Es fundamental que los criterios diagn&oacute;sticos sean homog&eacute;neos, de lo contrario las comparaciones entre diferentes estudios no son confiables. Este aspecto ha sido muy se&ntilde;alado por el grupo N&oacute;rdico, con gran experiencia en estudios colaborativos entre diferentes pa&iacute;ses<sup>(42-44)</sup>. Otro aspecto que ha sido especialmente destacado &uacute;ltimamente la importancia de que los estudios sean realizados en centros con experiencia en patolog&iacute;a pedi&aacute;trica<sup>(45-47)</sup>. </font></p>     <p align="justify"><font face="Verdana" size="2">Se debe recordar que la sofocaci&oacute;n en la infancia (accidental o por homicidio) con frecuencia no tiene elementos caracter&iacute;sticos en la autopsia<sup>(39,42,48-52)</sup>. Por otro lado, se han descrito lesiones histol&oacute;gicas muy discretas que se han vinculado a situaciones de violencia<sup>(39,53-57)</sup>. </font></p>     ]]></body>
<body><![CDATA[<p align="justify"><font face="Verdana" size="2">La revisi&oacute;n de las circunstancias de la muerte y la historia cl&iacute;nica son criterios de tal importancia que est&aacute;n incluidos en la definici&oacute;n. La discusi&oacute;n del equipo multidisciplinario es una etapa relevante en el estudio de las MIL, especialmente para el diagn&oacute;stico de SMSL. </font></p>     <p align="justify"><font face="Verdana" size="2">El protocolo utilizado se ajusta a los modelos de otros grupos<sup>(58-65)</sup>. Si la metodolog&iacute;a y los criterios diagn&oacute;sticos no son similares, los resultados no son comparables. </font></p>     <p align="justify"><font face="Verdana" size="2">Muchas enfermedades en el ni&ntilde;o pueden dar una muerte inesperada. Por otro lado, el hallazgo de lesiones menores insuficientes para causar la muerte no excluyen el diagn&oacute;stico de SMSL<sup>(43)</sup>. Entre las patolog&iacute;as que pueden causar la muerte se destacan las siguientes: </font></p> <ul>       <li>         <div align="justify"><font size="2" face="Verdana" align="justify"><b>Enfermedades card&iacute;acas:</b> a) cardiopat&iacute;as cong&eacute;nitas, &eacute;stas pueden dar cuenta de alrededor de un 13% de las MIL<sup>(66-71)</sup>; b) miocarditis de acuerdo al criterio de Dallas<sup>(72)</sup>, se observa en 6,6% de las ME y 4,3% del total de casos, las diferentes series han encontrado incidencia variable de acuerdo a los diferentes centros hasta un 28%<sup>(42,43,67,73,74)</sup>; c) anomal&iacute;as coronarias<sup>(67,75-79)</sup>; d) miocardiopat&iacute;as<sup>(80-86)</sup>; e) tumores card&iacute;acos<sup>(49,67,87,88)</sup> que pueden pasar desapercibidos en el examen macrosc&oacute;pico del coraz&oacute;n; f) otras patolog&iacute;as card&iacute;acas. Los trastornos del ritmo pueden ser causa de muerte y pueden tener o no un substrato morfol&oacute;gico. Es conveniente, antes de concluir una autopsia como negativa, realizar estudio del sistema de conducci&oacute;n<sup>(89-91)</sup>. El intervalo QT prolongado, que es causa de muerte s&uacute;bita<sup>(92)</sup> no se puede identificar en la autopsia. La esteatosis mioc&aacute;rdica es m&aacute;s frecuente en enfermedad metab&oacute;lica pero tambi&eacute;n puede verse en otras patolog&iacute;as<sup>(93)</sup>. La necrosis mioc&aacute;rdica en el reci&eacute;n nacido es el resultado, en general, de una lesi&oacute;n hip&oacute;xico-isqu&eacute;mica sin alteraci&oacute;n coronaria. Pasado el per&iacute;odo neonatal en general se debe a anomal&iacute;a coronaria. En la presente serie uno de los casos corresponde a una necrosis extensa mioc&aacute;rdica en un reci&eacute;n nacido pr&oacute;ximo al mes de vida con sufrimiento perinatal; otro de los casos corresponde a una placa fibrosa con compromiso del sistema de conducci&oacute;n relacionado con hipoxia perinatal. </font></div>   </li>       <li>         <div align="justify"><font face="Verdana" size="2"> <b>Causa respiratoria:</b> a) infecci&oacute;n respiratoria baja; b) asfixia; c) aspiraci&oacute;n. Un comentario especial merece esta categor&iacute;a, ya que la aspiraci&oacute;n de v&oacute;mito en nuestro medio figuraba como segunda causa de muerte postneonatal en la infancia. En esta serie se acepta como causa s&oacute;lo en tres casos; se cataloga como patolog&iacute;a asociada en ni&ntilde;os fallecidos por otra causa en ocho casos y usamos el mismo criterio en dos casos de ZG con elementos de aspiraci&oacute;n y en dos casos de AN (SMSL). <body><![CDATA[<div align="justify"><font face="Verdana" size="2"> <b>Infecciones.</b> Aparte de las respiratorias y del sistema nervioso ya consideradas, la sepsis dio cuenta de 9,3% de las ME de esta serie, y del 6% del total de la casu&iacute;stica. En la experiencia de otros grupos, esta patolog&iacute;a ha dado cuenta del 8% de los casos de MIL<sup>(45)</sup>. </font></div>   </li>       <li>         <div align="justify"><font face="Verdana" size="2"> <b>Traumatismo o maltrato.</b> La presente serie tuvo un caso de traumatismo enc&eacute;falo-craneano (con hemorragia retiniana concomitante). Las diferentes series coinciden en una incidencia no superior al 4% de esta patolog&iacute;a entre las muertes inesperadas de los lactantes.</font></div>   </li>       <li>         <div align="justify"><strong><font face="Verdana" size="2">Enfermedad metab&oacute;lica.</font></strong><font face="Verdana" size="2"> La deficiencia de Acil-CoA deshidrogenasa de cadena media es la que con mayor frecuencia puede manifestarse como una MIL. La autopsia puede tener una esteatosis hep&aacute;tica, card&iacute;aca y/o muscular, o no tener ninguna alteraci&oacute;n histol&oacute;gica. </font></div>   </li>     </ul>     <p align="justify"><font face="Verdana" size="2">A pesar de las m&uacute;ltiples investigaciones realizadas por diferentes grupos, no existen elementos patognom&oacute;nicos del SMSL<sup>(49,94-110)</sup>. La autopsia debe ser cuidadosa<sup>(62,111-114)</sup> y mesurada la interpretaci&oacute;n de las alteraciones encontradas<sup>(49,115)</sup>. El SMSL no es una entidad nosol&oacute;gica y constituye un diagn&oacute;stico de exclusi&oacute;n. </font></p>     <p align="justify"><font face="Verdana" size="2">En algunos ni&ntilde;os fallecidos s&oacute;lo se encuentran alteraciones m&oacute;rbidas menores que en s&iacute; mismas no justifican la muerte, catalogados como zona gris. Quedan interrogantes hasta qu&eacute; punto estas lesiones no son de alguna manera contribuyentes. Investigadores de gran prestigio en el tema<sup>(44)</sup> han expresado que la gran interrogante a ser aclarada ser&iacute;a &iquest;por qu&eacute; individuos (adultos o ni&ntilde;os) con la misma enfermedad pueden tener diferentes reacciones (ignorarla, enfermar y curar, enfermar y morir, y morir en el primer s&iacute;ntoma)? La respuesta estar&iacute;a en poder definir los casos de la zona gris, o sea aqu&eacute;llos que han fallecido con una causa m&oacute;rbida, pero que no es suficientemente importante para justificar la muerte desde el punto de vista an&aacute;tomo-patol&oacute;gico. </font></p>     <p align="justify"><font face="Verdana" size="2">En la presente serie se encontraron granulomas sin identificaci&oacute;n de germen en numerosos casos examinados. En un caso, en que se hizo PCR para BCG en el material en parafina, se confirm&oacute; que se trataba de unaBCG diseminada. Ten&iacute;a entre m&uacute;ltiples &oacute;rganos afectados, lesi&oacute;n de tronco encef&aacute;lico y se consider&oacute; causa de muerte. Se est&aacute;n procesando los estudios en los casos restantes. En los pa&iacute;ses donde se vacuna conBCGse han registrado estas observaciones<sup>(116-118)</sup>. </font><font face="Verdana" size="2"><b> </b></font></p> <font face="Verdana" size="2"><b> </b></font>     <p align="justify"><font size="3" face="Verdana"><b>    ]]></body>
<body><![CDATA[<br> Conclusiones</b></font></p>     <p align="justify"><font face="Verdana" size="2">La realizaci&oacute;n sistem&aacute;tica de la autopsia protocolizada por pat&oacute;logo pediatra y forense ha permitido conocer la verdadera causa de muerte en los lactantes de Montevideo que han fallecido inesperadamente. </font></p>     <p align="justify"><font face="Verdana" size="2">La mayor&iacute;a tuvo una causa m&oacute;rbida que justific&oacute; la muerte. Siendo la infecci&oacute;n respiratoria la causa mayoritaria, fundamentalmente en &eacute;poca fr&iacute;a. </font></p>     <p align="justify"><font face="Verdana" size="2">La causa violenta no tuvo relevancia en la muestra analizada. </font></p>     <p align="justify"><font face="Verdana" size="2">El s&iacute;ndrome de muerte s&uacute;bita del lactante pudo ser definido por los par&aacute;metros exigidos internacionalmente de autopsia negativa, investigaci&oacute;n del escenario de la muerte, historia cl&iacute;nica y discusi&oacute;n interdisciplinaria. </font></p>     <p align="justify"><font face="Verdana" size="2">Constituye el 20% de la muestra. </font></p>     <p align="justify"><font face="Verdana" size="2">El gran n&uacute;mero de ni&ntilde;os fallecidos por neumon&iacute;a, diarrea con deshidrataci&oacute;n, desnutrici&oacute;n, patolog&iacute;a card&iacute;aca, aportan datos de inter&eacute;s al equipo de salud que les asiste, quienes podr&aacute;n planificar pol&iacute;ticas de salud locales. </font></p>     <p align="justify"><font face="Verdana" size="2">Se resalta la importancia de las reuniones cl&iacute;nico-patol&oacute;gicas en los Centros de Salud con los resultados de las autopsias para devolver la informaci&oacute;n, as&iacute; como las reuniones de retorno con los diferentes servicios pedi&aacute;tricos del CHPR donde estos ni&ntilde;os estuvieron alguna vez internados. </font></p>     <p align="justify"><font face="Verdana" size="2">El desarrollo del programa MIL ha permitido tomar un tema que en el Uruguay estaba limitado a los archivos judiciales y ponerlo en la mesa de discusi&oacute;n pedi&aacute;trica para su aplicaci&oacute;n en la docencia, investigaci&oacute;n y planificaci&oacute;n de pol&iacute;ticas de salud, siguiendo los criterios actualmente sugeridos por la literatura internacional<sup>(45,115,119-122)</sup>.</font><font face="Verdana" size="2"><b> </b></font></p> <font face="Verdana" size="2"><b> </b></font>     <p align="justify"><font size="3" face="Verdana"><b>    ]]></body>
