Artificial intelligence is radically restructuring the global education system, driving a transition toward more personalized pedagogical models. Thus, this research aimed to critically analyze application trends and structural challenges of AI in Peruvian universities. The study followed a quantitative approach with a descriptive scope. The process rigorously adhered to the PRISMA protocol phases. Thirty-five (35) studies were included: 74.3% focused on Lima, highlighting centralization. Only 8.6% were indexed in Scopus/WoS, limiting international visibility. Non-experimental methodologies predominated (91.4%), with a quantitative emphasis (68.6%). Significant disparities were reported: 62.9% identified low digital skills among students, and 45.7% cited inadequate infrastructure-especially in rural areas (45% lack broadband)-hindering equitable applicability. In conclusion, AI integration in Peruvian higher education advances through fragmented initiatives, marked by a clear dichotomy between technical potential and structural barriers.
