Inflammatory paradental cyst (IPC) is a rare odontogenic lesion usually associated with partially erupted or impacted mandibular third molars. Radiographically, it presents as a well-circumscribed radiolucent image and can be clinically confused with a dentigerous cyst. This article presents the case of an 18-year-old patient with a symptomatic, horizontally positioned mandibular third molar, with a histopathologically confirmed diagnosis of IPC. The clinical, radiographic, and histological features are discussed, as well as the importance of early differential diagnosis for appropriate management.
