Radicular cysts are of odontogenic epithelial origin and occur due to inflammatory causes, as a result of pulp necrosis; they represent approximately 60 % of all odontogenic cysts. Decompression is a conservative technique for cystic lesions of the maxillary bones of considerable size. It consists of creating a communication between the cyst and the oral cavity by anchoring a tube or stent for drainage and continuous washing. The clinical case of a 24-year-old male patient with a large radicular cyst in the upper jaw with several years of evolution is presented; a combined treatment was performed, using the decompressive technique for 9 months and subsequently enucleation was performed along with guided bone regeneration, with good evolution.
