Hiatal hernia is a complication of the digestive tract that is usually rare, it is generally diagnosed intraoperatively, especially when the stomach is involved in the condition, this being the one found in the thoracic cavity. Hiatal hernias were described by Henry Bowdith who published on this complication between 1610 and 1846 for the first time and in 1848 Vincet Alexander Bochdalek described intestinal content in the lumbodorsal region of the diaphragm for the first time, giving rise to the diaphragmatic hernia. The definition of a hiatal hernia includes what is the protrusion or prolapse of the proximal stomach or any non-esophageal structure through the esophageal hiatus, which can be paraesophageal or sliding. The classic symptomatology involves dysphagia and symptoms similar to gastroesophageal reflux, in some patients it can be asymptomatic. The present case has an exceptional presentation, because it presents with a secondary problem of having an acute abdomen due to exacerbated cholecystolithiasis and a secondary gastric perforation; The elderly patient with comorbidities had a good postoperative evolution.
