Thyroid hormones affect the kidneys, heart, and vascular system, and their dysfunction signrficantly impairs renal function. They influence kidney development and growth, sodium and water homeostasis, renal plasma flow, and glomerular filtration rate (GFR). A decrease in thyroid hormone leads to reduced renal plasma flow and GFR, altering urinary concentration and dilution.

Chronic kidney disease and the uremic environment can lead to perturbations in thyroid status through multiple pathways. Most circulating thyroid hormone is protein-bound, and large protein losses in nephrotic syndrome or from dialysate in peritoneal dialysis patients can lead to total body thyroid hormone depletion.

Although the mechanistic link between thyroid disease and renal disease requires further study, evidence suggests a bidirectional relationship between hyperthyroidism and chronic kidney disease.
