A case report for the first Cerebral ByPass in Bolivia of a giant aneurysm arising from the right internal carotid artery, cavernous segment, treated with a high flow radial artery graft from the external carotid artery to the right middle cerebral artery M2-M3, and subsequent occlusion and exclusion of the giant aneurysm. The steps of the surgery are described and the difficulties of the technique and the benefits of the surgery are highlighted. The elements to be taken in consideration in the indication and realization of the bypass surgery in the treatment of the ICA giant aneurysm are discussed.
