The objective of this article is to report a case of complicated pleural empyema observed in the National Thorax Institute of La Paz Bolivia, in a 21-year-old male patient diagnosed with left pleural effusion, left thoracic trauma, left pulmonary contusion, hemoptysis of origin to be determined. Pleural empyema is a collection of pus in the pleural cavity, is more common in children, is more common in men and its incidence is higher in patients with a history of alcoholism, diabetes mellitus, rheumatoid arthritis and chronic lung disease. In addition, the pathophysiology and treatment based on the control of the infection by installation of the pleural system to the water seal will be exposed, and surgical options will be open or minimally invasive by thoracoscopy.
