Infective endocarditis during pregnancy is a rare event, but with a high rate of maternal and perinatal mortality. This poses a great challenge, both with regard to clinical management and to surgical treatment. The present case demonstrates the importance of multidisciplinary management. We present the case of a first pregnancy, with infective endocarditis due to Streptococcus pyogenes, whose outcome was the death of the product as a consequence of prematurity. The patient underwent valvular change due to severe mitral valve compromise; later I present a second gestation that concluded favorably.
