Case report: 29 year-old male patient, from La Paz city, without relevant pathologic antecedents, with clinical manifestations of 8 hours with progressive consciousness impairment associated to right brachiocrural hemiplegia, an important hypodensity is found in brain tomography, decompressive craniectomy is performed, and after tracheostomy and 23 days of intensive treatment, patient is discharged with Rankin 4. Discussion: This case presents important characteristics, such as the early age, hyperhomocisteinemia and anatomic lesion in left subclavian vein, it's worth mentioning in this case decompressive craniectomy, mostly underutilized in our country and that constitutes, without any doubt, a feasible alternative to save life and to preserve function in severe neurovascular disease.
