Abstract

Introduction: Stroke is not just an important cause of death, but also represents a cause of new life forms related to its incapacity. The hemispheric infarct resulting of acute occlusion of internal carotid or middle cerebral artery, represents a devastating subgroup that comprehends around 10% of ischemic stroke. The objective is report the case of a patient treated with decompressive craniectomy, fortunately with favorable evolution. Case report: 29 year-old male patient, from La Paz city, without relevant pathologic antecedents, with clinical manifestations of 8 hours with progressive consciousness impairment associated to right brachiocrural hemiplegia, an important hypodensity is found in brain tomography, decompressive craniectomy is performed, and after tracheostomy and 23 days of intensive treatment, patient is discharged with Rankin 4. Discussion: This case presents important characteristics, such as the early age, hyperhomocisteinemia and anatomic lesion in left subclavian vein, it's worth mentioning in this case decompressive craniectomy, mostly underutilized in our country and that constitutes, without any doubt, a feasible alternative to save life and to preserve function in severe neurovascular disease.
