Abstract

Introduction: Papillary carcinoma is the most freguent thyroid gland malignant tumour. Most of these tumours sem to be of low risk (89%) high healing and low mortality (1,8%). Distant and particulary brain metastases, and very rare. Caso report: A 52-year old woman with a short time evolution of raised intracranial pressure symptoms, including vomiting, headache and malasse that tended to worse. She had a II-III grade untreated goiter. A gadolinium-contrast MRI exam, showed homogeneously enhanced thyroid masses with perilesional oedema. A histopathology specimen examination, showed classic papillary thyroid carcinoma, tall cell and columnar cell variant. Twelve days after surgery, the patient died. Discussion: Metastases of papillary thyroid carcinoma usually occur to regional lymph nodes. Brain metastases were noted in 0.1-5% of patients. This sort of evolution is related to aggressive variants of carcinoma, and to old patients. At present time, therapeutic modalities are being discussed.
