Appears the case of a patient man, of 41 years of age, with previous diagnosis of chronic rheumatic cardiopathy, that attend with episodes of supraventricular paroxistic tachycardia, deal with amiodarone per prolonged time, with adapted initial answer to this treatment, that later presents recurrent episodes of paroxistic tachycardia without answer to the administration of amiodarone associated to lost of accelerated weight, fine distal tremor, perspiration and intolerance to the heat, being this clinical picture of recent appearance, without previous antecedent of thyroid pathology. It is dealt with propanolol, propitiouracilo and corticoids with evident improvement of the clinical picture in short time; metering of thyroid hormones, antibodies is antiRTSH and centellografy that confírms the proposed diagnosis. The methodology is discussed diagnoses and the therapeutic options in these cases.
