Star anise (Illicium verum) is utilized as a home remedy for gastrointestinal disorders in children and there is a risk of toxicity and lethality with this product. A 9 month old infant required hospitalization in the pediatric intensive care unit after ingestion of two liters of star anise. He refused to feed, was irritable, had respiratory difficulty and respiratory acidosis, tonic clonic convulsions, upper GI bleed and oliguria. It is recommended to avoid the use of star anise infusions in infants, due to the risk of complications such as the ones described in our patient.
