Resumen

MARIELA, Costas; SANTOS, Shelley; GODOY, Carolina  y  MARTELL, Miguel. Evolution of breast suckling in full-term and preterm newborn. Rev. bol. ped. [online]. 2006, vol.45, n.1, pp.54-68. ISSN 1024-0675.

The purpose of this work is to study the evolution of nutritive suckling aiming to know the association of variables related with suckling that allow an objective evaluation of it. It is a longitudinal study, prospective, in two groups of children. A group of full-term newborns, with adequate birth weight, followed since the first day to six months. The volume of milk intake was measured by double weight, and a register of intraoral pressure was made. This register was made putting a catheter connected to a transducer next to the nipple. There were made 169 registers. The volume of milk transferred to the infant has an significant initial increase of 0,01 to 0,2 ml in the early 7 to 15 days, then raises more gradually to 60-90 days, later the increase is slower. It is proposed that the flow of maternal milk causes the initial increase, then the infant have longer rounds and shorter time of pause, which would produce the increase in this second stage. Finally, the number of sucklings and the permanent increase of intraoral pressure make happen the final increase. The preterm newborns were studied between 33 and 41 weeks, it was used the same procedure employed in the previous group in the breast feeding, and when there were bottle feeded it was used a rubber teat with two perforations: in one was placed a catheter joined to a transducer and the other was used for the egress of milk with an outflow/minute of 21 ml. They extract more milk for bottle until 34 weeks, then the transference increase in both, but there is no significative difference. It is concluded that the register of objective variables as intraoral pressure and double weight allow to obtain information that helps to better know the evolution of breast suckling and to produce new strategies for maintaining an effective breast feeding.
