Gingival recessions are both an aesthetic and functional concern for the patient. They rarely result in tooth loss; however, they are usually associated with conditions such as tooth sensitivity and marginal tissue irritation. The literature has provided insight into various management techniques, including subepithelial connective tissue grafting combined with a coronally advanced flap. We present the case of a patient with a 20-year history of a palatal subepithelial connective tissue graft combined with a coronally advanced flap, generating functional and aesthetic attached gingiva around the tooth. The tissues remained healthy during 20 years of follow-up. This case confirms the efficacy and longevity of subpedicled connective tissue grafting with a coronally advanced flap.
