We report a case of McKittrick-Wheelock Syndrome with an atypical presentation. The patient was a 60-year- old male who presented with a left inguinal mass with signs of inflammation, accompanied by diarrhea of approximately one year's duration. Imaging studies revealed a rectal tumor causing near-total lumen stenosis that extended into the inguinal region, corresponding to the mass observed during the initial physical examination. Subsequent histopathological analysis identified the tumor as a non-keratinizing squamous cell carcinoma. Upon admission, persistent hypokalemia was confirmed, and despite initial management with electrolyte replacement and antibiotic therapy, the patient passed away before definitive surgical intervention. This case highlights the importance of considering this syndrome in patients presenting with clinical features similar to those described and underscores the need for early diagnosis due to its high mortality rate.
