A Pregnancy includes several physiological, anatomical, and hormonal changes, so in pathologies such as an acute abdomen, it is difficult to know the affected organ. A clinical case is presented of a 43-year-old patient treated at the Hospital Obrero N2, with a gestation of 33 weeks by second trimester ultrasound, who reports intermittent, oppressive abdominal pain at the level of the right fosaiiliac, for which a initiation by the acute appendicitis surgery service due to clinical signs because the patient does not present the characteristics of the condition in terms of laboratory and clinical signs. An exploratory laparotomy was performed because the patient continued to have acute pain that did not stop with analgesics and a twisted epiploic appendix was found located in the right iliac fossa with a necrotic appearance and a cecal appendix without alterations. makes the physical examination during pregnancy more difficult and confusing.
