Objetivos: Determinar la contaminación bacteriana en barbijos utilizados por estudiantes de medicina mediante el aislamiento e identificación de Staphylococcus aureus y bacterias de la familia Enterobacteriaceae. Métodos: Estudio descriptivo, cuantitativo de corte transversal en Cochabamba, Bolivia. Se analizaron 18 barbijos quirúrgicos de estudiantes de primer a tercer año de medicina mediante muestreo por conveniencia en julio de 2023. Se realizaron cultivos en agar sangre, tinción de Gram y pruebas bioquímicas para identificación bacteriana. La susceptibilidad antimicrobiana se evaluó mediante difusión en disco según criterios CLSI, incluyendo detección de resistencia a meticilina y Beta-lactamasas de espectro extendido. Resultados: 83,3% indicó un uso menor a un día. Se identificaron cocos Gram positivos en todas las muestras [100% (IC95%: 81,5-100)], predominando la familia Staphylococcaceae en 17 barbijos [94,4% (IC95%: 72,7-99,9)] y una cepa resistente a meticilina. Se identificaron bacilos Gram negativos en 33,3% [IC95%: 13,3-59,0] de las muestras, incluyendo enterobacterias, sin detectarse cepas productoras de Beta-lactamasas de espectro extendido. Conclusiones: Los barbijos quirúrgicos utilizados por estudiantes de medicina pueden albergar bacterias con resistencia antimicrobiana y enterobacterias, incluso tras períodos cortos de uso. Los hallazgos del estudio no permiten corroborar la hipótesis de una mayor contaminación asociada a un tiempo de uso más prolongado. Las limitaciones del estudio son el reducido tamaño muestral, tipo de muestreo y el diseño transversal que impide establecer relaciones causales.<hr/>ABSTRACT Following the COVID-19 pandemic, the widespread use of face masks raised interest in the microbiological changes occurring on their surfaces during prolonged use. Objectives: To determine bacterial contamination in face masks used by medical students through the isolation and identification of Staphylococcus aureus and bacteria belonging to the Enterobacteriaceae family. Methods: A descriptive, quantitative, cross-sectional study was conducted in Cochabamba, Bolivia. 18 surgical face masks used by first- to third- year medical students were analyzed using convenience sampling in July 2023. Blood agar cultures, Gram staining, and biochemical tests were performed for bacterial identification. Antimicrobial susceptibility was assessed by disk diffusion according to CLSI criteria, including the detection of methicillin resistance and extended-spectrum Beta-lactamase production. Results: A total of 83.3% of participants reported using their face mask for less than one day. Gram-positive cocci were identified in all samples [100% (95% Cl: 81.5-100)], with the predominance of the Staphylococcaceae family in 17 masks [94.4% (95% Cl: 72.7-99.9)] and one methicillin-resistant isolate. Gram-negative bacilli were identified in 33.3% of samples [95% Cl: 13.3-59.0], including Enterobacteriaceae, with no extended-spectrum Beta-lactamase-producing strains detected. Conclusions: Surgical face masks used by medical students may harbor antimicrobial-resistant bacteria and Enterobacteriaceae, even after short periods of use. The findings do not support the hypothesis of greater contamination associated with longer mask use. Study limitations include the small sample size, convenience sampling, and the cross-sectional design,which predudes causal inference. <![CDATA[EFECTOS DE UNA INTERVENCIÓN EDUCATIVA SOBRE EL USO DE PRUEBAS RÁPIDAS PARA DETECCIÓN DE ANTICUERPOS CONTRA EL VIH: UN ESTUDIO CUASIEXPERIMENTAL]]> http://www.scielo.org.bo/scielo.php?script=sci_arttext&pid=S1817-74332026000100011&lng=es&nrm=iso&tlng=es RESUMEN Objetivos: Analizar el efecto del uso de una charla educativa en la percepción de los conocimientos, actitudes y prácticas de los estudiantes de medicina con relación a la prueba rápida para detección de anticuerpos contra VIH. Materiales y métodos: Se aplicó un diseño cuasiexperimental, intragrupo, en varones y mujeres, mayores de edad, matriculados en la Facultad de Medicina de una universidad privada del Paraguay en 2025. Se aplicó un muestreo no probabilístico, de casos consecutivos. La intervención consistió en una charla educativa de expertos del Programa Nacional de Control del VIH/sida e ITS. Las variables se recolectaron con cuestionarios anónimos, antes y después de la charla. Se aplicó estadística analítica para comparar las medidas de tendencia central y de dispersión en los dos momentos. Se respetaron los aspectos éticos. Resultados: El cuestionario fue completado antes de la charla por 88 estudiantes y posterior a la misma por 96. La mayoría (31,82%) eran estudiantes del Io semestre. La mediana del cuestionario mostró mejoría significativa en el nivel de conocimientos y actitudes luego de la charla, excepto en la dimensión de las prácticas del uso de la prueba rápida para detección de VIH. Conclusiones: La charla educativa sobre el uso de la prueba rápida para detección de VIH demostró efectividad para mejorar el nivel de las dimensiones de conocimientos y de actitudes de los estudiantes de medicina, excepto en la dimensión de las prácticas. Se recomienda aplicar esta estrategia didáctica en forma continua.<hr/>ABSTRACT Objective: To analyze the effect of use of an educational talk on medical students' perceptions of their knowledge, attitudes, and practices regarding rapid HIV testing. Material and methods: A quasi-experimental, within-group design was applied to adult male and female students enrolled in the Faculty of Medicine of a private university in Paraguay in 2025. We applied a non-probabilistic sampling of consecutive cases. The intervention consisted of an educational talk by experts from the National HIV/AIDS and STI Control Program. Variables were collected using anonymous questionnaires before and after the talk. Analytical statistics were applied to compare measures of central tendency and dispersion at the two time points. Ethical considerations were respected. Results: The questionnaire was completed by 88 students before the talk and by 96 afterward. The majority (31.82%) were first-semester students. The median questionnaire score showed a significant improvement in knowledge and attitudes after the talk, except in the dimension of practices related to the use of rapid HIV testing. Conclusions: The educational talk on the use of rapid HIV testing proved effective in improving the knowledge and attitudes of medical students, except in the dimension of practices. It is recommended that this teaching strategy be applied continuously. <![CDATA[SÍNDROME DE GUILLAIN-BARRÉ ASOCIADO A PERITONITIS POR APENDICITIS PERFORADA Y FÍSTULA ENTEROCUTÁNEA: REPORTE DE CASO]]> http://www.scielo.org.bo/scielo.php?script=sci_arttext&pid=S1817-74332026000100018&lng=es&nrm=iso&tlng=es RESUMEN La aparición del síndrome de Guillain-Barré (SGB) posterior a infecciones abdominales es infrecuente y escasamente documentada, especialmente en pacientes pediátricos. El objetivo de este reporte es describir la asociación entre el SGB y una peritonitis generalizada secundaria a apendicitis perforada complicada con fístula enterocutánea. Se presenta el caso de un paciente masculino de ocho años que requirió múltiples intervenciones quirúrgicas para el control del foco séptico. Durante su evolución posoperatoria desarrolló debilidad muscular ascendente progresiva y arreflexia, hallazgos que condujeron al diagnóstico de SGB subtipo AMAN mediante estudios electrofisiológicos. El manejo incluyó control quirúrgico de la infección, antibioticoterapia dirigida, soporte nutricional, rehabilitación temprana e inmunoterapia específica, con evolución favorable. Este caso resalta la importancia de considerar el SGB ante la aparición de debilidad neuromuscular y arreflexia durante la evolución de infecciones abdominales graves, favoreciendo un diagnóstico y tratamiento oportunos.<hr/>ABSTRACT The occurrence of Guillain-Barré syndrome (GBS) following severe abdominal infections is uncommon and rarely reported, particularly in pediatric patients. The aim of this report is to describe the association between GBS and generalized peritonitis secondary to perforated appendicitis complicated by an enterocutaneous fistula. We present the case of an eight-year-old male patient who required multiple surgical interventions to control the septic source. During the postoperative course, he developed progressive ascending muscle weakness and areflexia, leading to the diagnosis of the AMAN subtype of GBS through electrophysiological studies. Management included surgical source control, targeted antibiotic therapy, nutritional support, early rehabilitation, and specific immunotherapy, with favorable clinical outcomes. This case highlights the importance of considering GBS when neuromuscular weakness and areflexia develop during the course of severe abdominal infections, facilitating timely diagnosis and treatment. <![CDATA[ÍLEO BILIAR SECUNDARIO A FÍSTULA COLECISTO-DUODENAL: REPORTE DE UN CASO Y REVISIÓN DE SU MANEJO LAPAROSCÓPICO]]> http://www.scielo.org.bo/scielo.php?script=sci_arttext&pid=S1817-74332026000100025&lng=es&nrm=iso&tlng=es RESUMEN El Íleo biliar es una complicación poco frecuente de litiasis vesicular, cursa con obstrucción intestinal mecánica. Se produce por la formación de una fístula colecistoentérica, por donde pasa el lito y se impacta en el íleon terminal, sitio más común a causa de su luz estrecha y peristaltismo reducido. Es más frecuente en pacientes de sexo femenino y mayores de 50 años. El presente caso trata de una paciente femenina de 50 años de edad quien acude a emergencia en marzo por cuadro de colecistitis aguda y rechaza procedimiento quirúrgico. 4 meses después retorna por el mismo cuadro y es intervenida quirúrgicamente de urgencia, donde se evidencia una fístula biliar y se repara la misma, en el postoperatorio presenta datos de obstrucción intestinal compatible con íleo biliar, por lo cual es reintervenida laparoscópicamente para la extracción del lito.<hr/>ABSTRACT Gallstone ileus is a rare complication of gallstones, characterized by mechanical intestinal obstruction. It occurs due to the formation of a cholecystoenteric fistula, through which the stone passes and becomes lodged in the terminal ileum, the most common site due to its narrow lumen and reduced peristalsis. It is more frequent in female patients and those over 50 years of age. This case involves a 50-year-old female patient who presented to the emergency department in March with acute cholecystitis and declined surgery. Four months later, she returned with the same symptoms and underwent emergency surgery, during which a biliary fistula was identified and repaired. Postoperatively, she presented with signs of intestinal obstruction consistent with gallstone ileus, requiring a second laparoscopic surgery for stone extraction.
